Simply provide your name, phone number, and credit card number. Seating is limited - call to save your seat NOW!!!  Registration is $299, $249 if registered 15 days in advance of seminar. Registration at the door if we still have room -(Don't count on it): $349.

A box lunch or buffet will be provided. No formal lunch break will be taken except time scheduled between lectures. Each hour will begin on time and is monitored by both our staff/administrator and by a doctor assigned as course monitor.

Each doctor attending the seminar will receive a certificate of attendance at the conclusion of the seminar or within one week of the seminar.
